訂房及入院

  • 訂房：必須由主診醫生代訂房間。

  • 請攜備身份證／護照及按金辦理入院登記，並必須 提供保證人及緊急聯絡人資料作記錄之用。 (十八歲以下病人請攜帶出生證明文件，並須由家長或 監護人陪同下登記。建議在兒科病室內由一位家長或 監護人陪伴病人。)

  • 辦理入院時，需要提供個人信息以作登記，包括但 不限於住址(不接受郵政信箱)、聯繫電話、出生日期、 性別、年齡、國籍、近親、擔保人(包括擔保人的住址 和身份證/護照號碼)。

  • 請勿攜帶大量現金及貴重物品。如有財物損失或損壞， 本院恕不負責。本院設有電子夾萬，供病人存放個人 財物，任何時候皆不應隨意放置手袋、現金、首飾、 手錶、個人電子產品／手提電話／電腦等物品。
